I authorize Hot Yoga West to process a debit, in paper, electronic or other form in the amount of 499:- (first month half price) and 999:- thereafter on the ____ day of each month.

• I agree to a 1-month minimum term.
• I understand that the Monthly AutoPay Membership allows me to practice unlimited yoga at Hot Yoga West, excluding special events and workshops.
• Once the 1-month term is complete, it will continue on a month to month basis until terminated. Notification must be  by email 15 days before the monthly payment is processed.
• I understand that once a month, I am allowed to bring a first-time guest to attend class at Hot Yoga West.
• I am eligible for 10% discount on all retail products excluding food items.
• I understand that I may freeze my contract once per year without a fee. Additional freezes can be applied at 15:- per day.
• For unlimited memberships Late cancel/No show policy 6 hours before booked class or incur late cancel penalty fee 200kr.
• If I incur 5 late cancel/no show bookings, I understand that my online booking privileges will be placed on a temporary suspension for 1 month. If I choose to remove the suspension right away I agree to a fee of 500kr.
• I acknowledge that there are no refunds or exchanges once a payment has been processed.
• Membership is non-transferable.
